A guillotine was apparently going to be used by The Cheat to decapitate Strong Bad in disconnected. Strong Bad was imagining the benefits of separating his head and his body, so that he could double his productivity. However, when he imagined Head Bad and Strong Body regenerating as "two perfectly-formed Strong Bad specimens," he decided that chopping his head off wasn't such a good idea.
It was seen again in The House That Gave Sucky Tricks when it was used by Homsar, who was apparently decapitated with it, but the scene cuts away just as the blade drops.
Actual guillotines were common execution devices during the French Revolution. The blade would be raised with a rope, and then dropped, severing the victim's head.
